The Cigna Group Unveils Lead to One and $3 Billion Plan at Investor Day

The Cigna Group used its 2026 Investor Day to introduce “Lead to One,” a vision built on personalization at scale. The health company also announced a $3 billion multi-year modernization and productivity initiative, according to a company press release.

The Cigna Group reaffirmed its 2026 financial guidance and set a long-term target of 10-14% adjusted EPS CAGR through 2030.

What Lead to One Means for The Cigna Group

“We are proud to announce ‘Lead to One,’ our vision for the future of health care through personalization at scale,” said Brian Evanko, President and Chief Executive Officer of The Cigna Group.

The company said the vision unifies all colleagues around personalization at scale to improve affordability and the patient experience. Leaders outlined a growth framework with three parts:

  • Deliver core growth across Cigna Healthcare and Evernorth’s Specialty & Care Services and Pharmacy Benefit Services
  • Leverage complex care capabilities enabled by clinical expertise, data, technology and AI
  • Execute with discipline through operational excellence and capital stewardship

The framework reflects a wider focus on strategic thinking in future leadership.

Complex Care at the Center

The company said patients with complex needs often require support across multiple conditions, specialty and traditional medications, behavioral health and ongoing care navigation.

Evernorth Specialty & Care Services supports more than one million patients with complex conditions, the company said. It also delivers more than 8 million prescriptions annually.

Evanko said the company is uniquely positioned in the fastest-growing areas of health care. He pointed to the combined capabilities of Evernorth Health Services and Cigna Healthcare.

New Grant Program

The Cigna Group Foundation announced a new $10.5 million, three-year grant program. It aims to expand community support for patients and caregivers and address non-clinical barriers to treatment.

$3 Billion Modernization Plan

The new $3 billion initiative will focus on modernizing processes and streamlining workflows. It also aims to give colleagues AI-enabled insights and tools and to tighten supplier and vendor management.

“Our long-term growth targets are supported by our clear growth pillars, distinct capabilities, and our new multi-year modernization and productivity initiative, which will self-fund and fuel our performance,” said Ann Dennison, Executive Vice President and Chief Financial Officer.

2026 Guidance and 2030 Targets

The Cigna Group reaffirmed 2026 consolidated adjusted revenues of about $280 billion and consolidated adjusted EPS of at least $30.45.

It expects Evernorth Health Services pre-tax adjusted income from operations of at least $6.90 billion. For Cigna Healthcare, the figure is at least $4.55 billion. The medical care ratio is guided at 83.7% to 84.7%.

Through 2030, The Cigna Group targets 10-14% adjusted EPS CAGR and about $50 billion in cumulative operating cash flow.
